Consultations are by appointment.

Please telephone on 9329 8103 or email using the form below for an appointment. Every effort will be made to accomodate your preferred appointment time. If you feel you need a longer appointment, please advise us at the time of making the appointment.

Home visits can be arranged for those patients whose condition prevents them from attending the surgery.

If you cannot make your scheduled appointment, please give at least 24 hours notice. This will allow other patients to be scheduled as required.

We are closed on public holidays.

Please understand that this is a request form only and not a confirmation. Your appointment is confirmed only once you have received a confirmation from the clinic.

You will be contacted as soon as possible and within 2 working days to confim your appointment. Please call us if your appointment request has not been acknowledged within 2 working days.

Please do not ask for test results or medical advice by email as these requests will be declined and a suggestion for an appointment to be made will be provided instead.
